New Psychology Client

Your child has an upcoming appointment with one of our psychologists. To ensure we provide the best possible care, please review the information below and complete the required documentation prior to your visit.

1. Check Your Confirmation Email

A confirmation letter has been sent to your email.

It contains essential details regarding your appointment: location, date, time, and fees

3. Complete Required Forms

We require both these forms to be submitted before your arrival to ensure a streamlined appointment:

4. External Agencies

If your child is cared for by an external agency such as DCJ, we also require you to complete our External Agency Patient Intake Form. This is emailed to you and needs to be completed prior to an initial appointment being booked.


If your child is funded by the NDIS, we require you to complete our NDIS Participant Intake Form and Service Agreement. This is emailed to you and needs to be completed prior to an initial appointment being booked.


Important: Cancellation Policy

We require at least two business days' notice for any cancellations or changes to avoid a fee. This allows us to offer the appointment time to another child on our waiting list.
