New Paediatric Patient

Your child has an upcoming appointment with one of our paediatricians. To ensure we provide the best possible care, please review the information below and complete the required documentation prior to your visit.

If your child is cared for by an external agency such as DCJ, we also require you to complete our External Agency Patient Intake Form. This will be sent to the email address provided.

Important: Cancellation Policy

We require at least two business days' notice for any cancellations or changes to avoid a fee. This allows us to offer the appointment time to another child on our waiting list.
